- Medical History: multiple strokes, Parkinson’s Disease, diabetes
- Prior Independence Level: lived at home with his wife, drove and was active in the community
- Hospitalization: after experiencing a decline in cognition and several falls he was hospitalized
Lemont
- Duration of stay: 6 weeks
- Admission Status: able to walk only 75 feet using a rolling walker and required moderate assistance with self care, transfers, communication and cognition
- Therapy Received: Physical, Occupational and Speech therapy
- Discharge Status: able to walk independently up to 300 feet and demonstrated significant gains in self care, transfers, communication and cognitive status
- Discharge Living Arrangements: with mild modifications to his environment he returned to independent living in his home.
